  By subscribing to our text messaging service, you consent to receive SMS and/or MMS messages from The HCG Institute related to promotions, updates, alerts, or other information. Message frequency may vary.

  By providing your mobile phone number and completing the opt-in process, you are expressly consenting to receive text messages from The HCG Institute. Consent to receive text messages is not a condition of purchase.

• Opt-Out
  You may opt out of receiving text messages at any time by replying with STOP,” “CANCEL,” or “UNSUBSCRIBE”. Upon opting out, you will receive a confirmation message, and no further messages will be sent.

  The HCG Institute is not responsible for any delays or undelivered messages due to carrier issues or other technical problems.

• Changes to Terms
  We reserve the right to modify or terminate our text messaging service or these terms at any time. Continued use of the service following any changes constitutes your acceptance of those changes.

Credit Card Authorization 

I understand that I am required to complete the medical intake form and/or bloodwork to be in compliance and continue with The HCG Institute's programs. I understand that the medical intake form and/or bloodwork are required to be completed before the provider can determine if treatment is medically necessary. I hereby authorize The HCG Institute to charge the credit card account.

The cardholder agrees to pay any amounts charged by The HCG Institute to this credit card and authorizes The HCG Institute to obtain credit approval from the provided credit company. I affirm that I am at least 18 years old and that I am legally authorized to use the credit card account used during online checkout.

The HCG Institute does not accept or bill insurance for any programs. Once a program is started, The HCG Institute cannot honor any refund requests based on scheduling conflicts, missed doses, or unsatisfactory results. I understand that payment is due in full at the time of service. I acknowledge that my provider has no contracts with any insurance company and is not contractually obligated to pre-certify treatment with my insurance company or answer letters of appeal. I have carefully read and initialed the preceding sections.

Furthermore, I understand and agree that any charges made to the account specified for products or services are non-refundable and cannot be cancelled even if I am denied service based upon my blood results. I understand that if a credit card charge is disputed a $150 fee will be assessed for validating the charge in addition to any collection or attorney fees to settle the debt. The HCG institute is not responsible for a delay in processing or receiving bloodwork by the desired laboratory. In addition, please note that the retrieval of bloodwork is the patient’s sole responsibility. I understand that I am free to discontinue participation in this treatment program at any time. I agree to the use of arbitration to settle legal controversies that may arise as part of my treatment program.
