  We reserve the right to modify or terminate our text messaging service or these terms at any time. Continued use of the service following any changes constitutes your acceptance of those changes.

Credit Card Authorization 

I understand that I am required to complete the medical intake form and/or bloodwork to be in compliance and continue with The HCG Institute's programs. I understand that the medical intake form and/or bloodwork are required to be completed before the physician consultation. After a prescription is uploaded and by purchasing and completing the checkout process I hereby authorize The HCG Institute to charge the credit card account.

The cardholder agrees to pay any and all amounts charged by The HCG Institute to this credit card and authorizes The HCG Institute to obtain credit approval from the provided credit company. I affirm that I am at least 18 years old and that I am legally authorized to use the credit card account used during online checkout.

The HCG Institute is not responsible for overnight prefilled medication if the patient is not available to receive the package on the day of delivery. Furthermore, I understand and agree that any charges made to the account specified for products or services cannot be canceled and are non-refundable even if I am denied service based on my blood results or non-receipt of blood results and/or the medical intake form. I understand that if a credit card charge is disputed a $150 fee will be assessed for validating the charge.

The HCG institute is not responsible for a delay in processing or receiving bloodwork by the desired laboratory. In addition please note that the retrieval of bloodwork is the patient's sole responsibility. The cardholder agrees that all litigation must be pursued in the city of Philadelphia in the state of Pennsylvania. I agree to pay pursuant to my agreement with said credit card company any such amounts charged to me by The HCG Institute for products or services performed or any combination of both.
